TOC: Ability to hide pages
Revert to the old table of contents where we could remove some pages (minus sign) in the TOC. We also need the refresh button back.

My proposal is in landscape, and the TOC is too long to fit on one page. So i had two TOC's side by side on one page, and was able to exclude items with the minus sign, so that the TOC would fit all pages in the doc on the one page. Please bring back that feature
